Vision Screening
Visual acuity critical-line screening for distance and near vision. Color deficiency screening for male students, no earlier than the first grade.
TK/Kinder, 2nd, 5th, 8th
Documentation
Screening results for each student will be shared digitally via secured link to district designee.
Hearing Screening
Pure Tone Audiometric Screening for both ears with a threshold rescreening in 2-6 weeks for failed initial screenings.

Referrals
Information needed to generate referrals for a complete evaluation by a health care provider will be provided to district designee.
